3 X PCN Nurse Practitioner – Population Health Management
Pay Scheme: Other – £42,842 per annum
Job type: Fixed term until 31 March 2022
Working Pattern: Full Time (37.5 hours per week)
Days/Times: Monday – Friday

We are looking to recruit three full time PCN Nurse Practitioners to work alongside member practices within Canvey Island, Rayleigh & District and Rochford District Primary Care Networks (PCN).
The successful individuals will have experience of working within General Practice as either a Practice Nurse or Nurse Practitioner with an interest in preventative chronic disease management.
This role will be working in partnership with the PCN Clinical Auditor and Healthcare Assistant to support chronic disease management. By introducing this service, patients will have easy access to a comprehensive health and wellbeing support for management of their chronic disease and creating greater patient satisfaction.
These roles will be leading on specific mandatory Quality Outcomes Framework (QOF) measures across the PCN which include:
- Cardiovascular disease
- Atrial Fibrillation
- Heart Failure
- Hypertension
- Peripheral Arterial Disease
- Stroke/TIA
- Diabetes
- Learning Disability
- Obesity
- Cancer Screening (cytology)
- Early Cancer Diagnosis
- COPD
This role will be a core member of the multidisciplinary team, working alongside other Clinical Pharmacists, pharmacy technicians, physician associates, mental health practitioners and Care Coordinators.
