About The Role

An exciting opportunity for forward thinking motivated Clinical Pharmacists and  Pharmacy Technicians to join the expanding multidisciplinary team working within Loughton, Buckhurst Hill and Chigwell Primary Care Network (LBC PCN).

We bring together seven GP practices and their patient populations from the Loughton, Buckhurst Hill and Chigwell areas.

Our population is on the edge of London with access to the London transport networks but is actually in Essex. Equally our patients have a mixture of flows of healthcare out into Essex as well as into London.

Main Job Summary
  • The Senior Practice PCN Pharmacist will work to develop pharmaceutical and medicines optimisation services, and contribute to the management of long term conditions. They will do this both by contributing directly to the objectives set out in the job descrption, and by developing and managing a team of less experienced Practice Pharmacists in General Practice.
  •  The post-holder will work with clinical colleagues to ensure the highest standards of care are provided for all registered and temporary patients across the PCN practices and to become a core member of the team of healthcare professionals.
  • The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, acute prescription request, while addressing both the public health and social care needs of patients in the GP practice(s).
  • The post holder will perform face to face medication reviews of patients with polypharmacy especially for older people, people in residential care homes and those with multiple co-morbidities, amongst other main job duties as outlined in the attached job description
About The Candidate
You will be a qualified and experienced Senior Clinical Pharmacist, ideally with strong experience within Primary Care and have the following skills / qualifications, see attached person specification for more information:
  • Masters Degree Pharmacy, and active registration with the General Pharmaceutical Council
  • Post qualification experience in pharmacy services, ideally in Primary Care
  • Working towards/being an independent prescriber
  • CPR Certificate
  • Member of the Royal Pharmaceutical Society (RPS)
  • You will have awareness of the breadth of common acute and long term conditions that are likely to be seen in a general medical practice
  • Be able to demonstrate general pharmaceutical skills and knowledge in core areas
  • You will be able to plan, manage, monitor, advise and review general care plans for patients with  long term conditions
  • Be able to demonstrate accountability for delivering professional expertise and direct service provision as an individuals
  •  Be able to recognise priorities when problem-solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs with confidence when appropriate
  •  Be able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct
  •  Assist patients in decisions about prescribed medicines and supporting adherence as per NICE guidance
  •  Mature and non-judgemental attitude and be flexible and adaptable
  • Self motivated with excellent communication skills
  • Understand the need for patient confidentiality
About the Surgery

At Loughton, Buckhurst Hill and Chigwell (LBC) PCN, we are developing forward looking models of care integrating health and social / wellbeing, concentrating on patients (and their carers) and their holistic needs.

The well-being and development of clinical and non-clinical staff working / attached to the network is a key priority for us.

The successful candidates will work across a number of practices developing these new and exciting roles closely supported by the GPs, pharmacists and the wider clinical team. The 7 practices in the PCN serve a population of approximately 60,000 patients.

The pharmaceutical team will help patients to get the best from their medicines through undertaking structured medication reviews, effective medicines reconciliation, electronic repeat dispensing, monitoring of high risk drugs, and handling of prescription queries within the areas of competence.
