About the Role

The Clinical Pharmacist will work with a team of Clinical Pharmacists to provide a comprehensive service for the PCN.

The postholder will work within their clinical competencies as part of a wider multi-disciplinary team to provide expertise in clinical medicines management, structured medication reviews (remote and face-to-face) with particular focus on patients with polypharmacy (especially for those with frailty and co-morbidities), manage long-term conditions, management of medicine on transfer of care and systems for safer prescribing to include repeat prescribing, repeat dispensing and acute prescriptions. They will be encouraged to have an area of expertise.

They will provide leadership on quality improvement and clinical audit, as well as managing aspects of medicine safety and antibiotic stewardship. They will contribute to practice financial optimisation of medicines related targets in QOF to include improving disease registers; requirements of the PCN DES and Local Enhanced Services support.

NB:  18.5 hours part time are the minimum amount of part-time hours for the role. 
About the Candidate

The successful candidate will have a patient facing role in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promote self-care. Supporting excellence in prescribing in accordance with best practice and targets will be essential to the role, as part of collective working.

The role is expected to develop over time with engagement and supervision. Experience in General Practice would be an advantage but is not essential.

The following criteria is essential - please see attached person specification for full person specification:

Pharmacy Degree and registration

Registration with General Pharmaceutical Council

Minimum of two years working as a pharmacist

An appreciation of the nature of GPs and general practice. An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing

In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare

Commitment to reducing health inequalities

Knowledge of needs of patients with long-term conditions

Able to recognise personal limitations appropriately and demonstrates accountability
