About The Role

Loughton, Buckhurst Hill and Chigwell PCN is recruiting.  We are looking to welcome Clinical Pharmacists into our busy PCN, who will involve playing a full and active role as part of the practice multidisciplinary team.

The main responsibilities of the role include:

  • Clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
  • Provide specialist expertise in the use of medicines and medicine information (i.e. side-effects, dosage) across the practice.
  • To provide structured medication review and health education advice to patients at the designated practices by face to face, telephone or by notes-based review as agreed with the practice.
  • See, (where appropriate), patients with single or multiple long term conditions where medicines optimisation is required (e.g. asthma, COPD, diabetes, frailty etc.)
  • Manage patients with minor/self-limiting ailments while working within a scope of practice and limits of competence.
  • Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate.
  • Provide leadership on person centred medicines, optimisation and quality improvement.
  • To contribute to public health campaigns, including flu vaccinations, childhood immunisation and adult immunisation programmes.
  • Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.
  • Development and implementation of new services.
  • Act as key contact in practice for all medication related queries.
  • To support improvements in clinical care through practice-based audits and implementing change.
  • To work closely with CCG pharmacists to aid prescribing decisions and manage near patient testing.
  • To actively participate in training and development opportunities and maintain CPD portfolio
About The Candidate

The ideal candidate will have the following skills:

  • Completion of Pharmacy Degree and post graduate experience.
  • Independent Prescriber status (or willing to work towards).
  • Excellent Clinical and Medication knowledge.
  • Experience of undertaking medication reviews.
  • The commitment to continue to learn and develop.
  • The ability to be adaptable and flexible and be a proactive problem solver.
  • Excellent inter-personal patient facing communication skills.
  • Registered with the General Pharmaceutical Council.

Experience within primary care is desirable but not essential as full training will be provided. Successful candidates will be expected to enrol on the CPPE Primary care pharmacy education pathway and complete an independent prescribing qualification. We are looking for team players with a willingness to learn and flexible and proactive approach. Full and part time applications welcome.

About Us:

Loughton, Buckhurst Hill and Chigwell PCN is formed of 7 practices, looking after around 60,000 patients in Redbridge and Epping Forest. We are one of 6 PCNs within West Essex CCG.

We are looking expand our vibrant PCN team in line with our vision of ‘supporting the ARRS workforce to develop a wide range of skills to provide services using a practice centred approach.’ These services include minor illness, long term condition reviews, vaccinations, ECGs and spirometry amongst others. Successful candidates will also be expected to assist practices in achieving QOF and DES targets as well as developing and implementing quality improvement projects. We work closely with the wider healthcare team including district nurses, specialist community teams, care homes and community pharmacies to provide high quality and person centred care.
